Outpatient Anticoagulation Clinic
Our Mission
Our mission is to provide safe and effective management of patients on Warfarin using a systematic and organized approach. Our team of nurse practitioners and supervising physician utilize integrated management software, patient education, and accurate point of care testing with dose adjustment according to current standards of practice to ensure the highest quality of care.
Discharged patients
It is the physician’s responsibility to provide short term care (medications and instructions) until the patient can be seen at the clinic. Patients sent home on LovenoxTM should be provided with proper education for self-administration. If there are any questions regarding the day of discharge dosing, please contact the supervising clinician at 574-647-1990.
Referral
A physician order is required for new patients to be seen at the Outpatient Clinic. Referral forms are available on the Memorial web site. Two forms are required, a patient information referral form and a physician referral agreement. Any assistance from nursing in coordinating the patient referral is much appreciated.

We are currently seeing patients by appointment, Monday through Friday from 8:00 a.m. - 4:30 p.m.
