Reaction to the 2010 Indiana Hospital Self-Reportable Events Update

Memorial Hospital of South Bend: Saving Lives

We have conducted extensive investigations on the self-reportable events that occurred in 2010. Our goal is to have no errors, however, we are grateful that all five patients recovered without complications, and with no permanent injury. We have used these investigations, which included root-cause analyses, to make appropriate changes to processes and policies.

At Memorial, we believe in transparency and open disclosure and regularly educate and train our staff on best practices to help avoid future reportable events. We are thankful for the ongoing commitments made by our medical staff to help us keep patients safe every day.
