Helping Patients Transition to Home: Memorial’s New Home Care Connect
Memorial Home Care’s new Home Care Connect is ideally situated to serve patients as they leave Memorial Hospital. Home Care Connect helps link patients and their families to the critical support services and products needed to transition back home safely. Visitors can purchase some common products that are readily available on site, as well as make arrangements for the delivery of larger items.
Home Care Connect opened in April and has been a clear success. “The patients benefit from the many medical products and services we offer, and visitors come in as well to see the other items, such as the sun-blocking clothing,” explains Rosemary Green, Home Care liaison.
Located on the first floor of Memorial Hospital across from the Cottage Place Gift Shop, Home Care Connect provides patients and their families with access to our full range of services, including:
- Nursing, Therapy and Social Work Visits
- Homemakers and Personal Care Attendants for Non-Medical Care
- Home Health Aides and Nurses
- Home Medical Equipment
- Home Infusion
Patients need not make an extra trip to arrange for any of these services; it’s all available to them and their families before they make the transition to home living. More information about Memorial Home Care and Home Care Connect is available at 574-647-8500.
