Prenatal Care Coordination Program

Prenatal Care Coordination (PNCC) is a home visit program that provides free services to high risk pregnant women. It is designed to help pregnant women stay healthy and to help babies be born healthy.

PNCC helps reduce barriers to early prenatal care, initiates enrollment in Hoosier Healthwise or medical referrals to WIC, food pantries, counseling, breastfeeding support education, parenting skills, support groups, pregnancy, childbirth and nutrition information.
