Community Health Programs and Services

There's more to a healthy community than treating illness. To be among the healthiest communities in the country, a good measure of the effort must be in prevention.

For years, Memorial has been recognized as much for our work in creating health as for our excellent clinical services. As one of the country's first hospitals to establish a tithing policy, Memorial funds several innovative and collaborative programs, some of which have served as national models for health outreach and preventive care.

Memorial's tithing program is above and beyond the traditional charity care provided for patients who cannot afford health services. Each year, a portion of our excess revenues are set aside to help local organizations develop innovative programs that address a vast range of risks to community health. Whether the issue is teen pregnancy, access to appropriate health services, early prenatal care or one of many other challenges, we are proud to help our community meet them, defeat them, and grow into the healthiest community in the nation.

  • BrainWorks

  • Community Health Alliance (CHA)
    A locally owned physician hospital organization (PHO) serving Indiana and Michigan, Community Health Alliance offers a network of carefully selected physicians, hospitals, and other healthcare providers who have joined forces in a cooperative effort to control cost while assuring that quality services are provided.
     
  • Community Health Enhancement
    Memorial is proud to offer innovative, community-based programs for a healthier quality of life that is second to none. Community Health Enhancement coordinates the Beds and Britches, Etc. (B.A.B.E.) program; BrainWorks; Women, Infants & Children (WIC); and many others for the benefit of our community.

    Downtown South Bend's only medically-based facility offering group exercise classes, a personalized assessment and orientation, swimming, indoor sports and more.
     
  • Health Discovery Center
    The Health Discovery Center provides important health, wellness and prevention information through experienced nurses and wellness counselors, as well as health screening services and group education sessions by appointment.

  • HealthWorks! Kids' Museum
    Memorial's interactive children's health education center and museum works to infectiously contaminate kids to learn, have fun and make great life choices.
     
  • Home Care
    Memorial Home Care is your one-stop shop for home health care, providing a full range of services including nursing, therapy, home health aides, and home infusion. Most of our services are available to 24 hours a day, 7 days per week.
     
  • Home Care Connect
    Memorial Home Care Connect helps link patients and their families to the critical support services and products needed to transition back home safely. Visitors can easily purchase common products and make arrangements for the delivery of larger items.
