Congestive Heart Failure (CHF) is one of the most prevalent diagnoses today as more people survive heart attacks and are left with residual heart problems. An extensive number of cases of congestive heart failure are treated within our program each year. The goal of our program is to improve quality, prevent unnecessary emergency room visits and hospital admissions, promote consistency in use of ace inhibitors-new medicines that improve cardiac output -and promote better self-management through patient education and lifestyle changes.
