Hoosier Healthwise

(Applicants must reside in Indiana)

Indiana's health care program for children from birth to 18 years of age, pregnant women, and low-income working families. The program is administered by the Indiana Family and Social Services Administration. Coverage includes primary and preventive care services. Volunteer Provider Network

With community partners, and the leadership of a group of physician champions, a system was developed to better coordinate health care for low income, medically uninsured individuals in St. Joseph County. The Volunteer Provider Network (VPN) provides access to essential health care, including primary physician services, specialty services, hospital services, x-rays, and lab work, at no charge to the enrollee while creating an equitable distribution of charity care for the providers in our community.

The VPN program is available to residents in St. Joseph County who live at or below 150 percent of the Federal Poverty Guidelines, do not have access to medical insurance and are not qualified for any form of public insurance. The VPN program receives administrative funding from Memorial Hospital and Saint Joseph Regional Medical Center.
