Our Mission:

Our mission is to provide safe and effective management of patients on Warfarin using a systematic and organized approach. Our team of certified anticoagulation pharmacists utilize integrated management software, patient education, and accurate point of care testing with dose adjustment according to current standards of practice to ensure the highest quality of care.

We are currently seeing patients by appointment on
Mondays, Wednesdays and Fridays from 8 a.m. - 4:30 p.m.
